UA Information

You can find out if you need to UA today by calling (541) 201-8770 or clicking on the button below. Both options will prompt you to enter your client ID number, if you do not know your client ID number call a staff person to get that information.

You can UA anywhere from 10:00AM – 4:00PM but it is recommended you contact the office at (541) 272-5048 to make sure there is a same-sex staff member available. If you are unable to complete a UA during that time frame, consult with your counselor and arrangements will be made to try and accommodate you.
